We performed early detection for diabetes and hypertension in the Salon Comunal (community center) in Micaela Bastidas. We invite participation from adults in the census who are 35 years and older. We use British Hypertension Society-approved devices for blood pressure determination. Type II diabetes is diagnosed by two abnormal fasting blood sugars. We measure blood sugar using an analyzer made by Hemocue that is FDA-approved for diagnostic use. From every patient we obtain responses to a questionnaire of risk factors for diabetes and cardiovascular disease and we measure height and weight (to calculate body-mass index). Our goal is to screen 80% of adults in the census. In order to achieve that goal we make home visits to anyone among the invited participants who does not show up for testing in the community center.
The Ethics Committee at Universidad Peruana Cayetano Heredia approved our proposal to develop a diabetes risk score from the data we collect as part of the early detection program. Our goal is a scoring system based on risk factors, body mass index and blood pressure that will detect 80% of diabetics while identifying only 20% as high-risk in need of glucose testing. In that way we can perform expensive glucose testing in 80% fewer patients and reduce the cost of our early detection program.
